September 28, 2021 (Lincoln, Neb.) – An inmate who went missing from the Community Corrections Center – Lincoln (CCC-L) earlier this month is back in custody. Zackery Carlstrom #212038 was arrested by members of the Fremont Police Department today and is being held in the Saunders County jail where he is facing a new charge of obstructing a peace officer.

September 23, 2021 (Tecumseh, Neb.) – An inmate assaulted a staff member at the Tecumseh State Correctional Institution (TSCI) today, resulting in a serious injury*.

The staff member was unexpectedly headbutted by the inmate, resulting in an injury to his nose. The staff member was assisted by other teammates in restraining the inmate. The injured staff member was treated at the Johnson County Hospital.

September 16, 2021 (Omaha, Neb.) – An inmate assaulted a staff member at the Nebraska Correctional Youth Facility (NCYF) today, resulting in a serious injury*.

The staff member was inside a classroom when the assault occurred. The inmate crossed the room and initially pushed the staff member in the chest. September 11, 2021 (Lincoln, Neb.) – Director of the Nebraska Department of Correctional Services (NDCS) Scott R. Frakes announced that an inmate in his 60s died today at a hospital in Lincoln. The inmate tested positive for COVID-19 prior to being hospitalized. In addition to the coronavirus, he had underlying medical conditions.

Frakes announced that an inmate in his 30s died Friday, September 10, 2021 at a hospital in Lincoln. The inmate tested positive for COVID-19 prior to being hospitalized. September 9, 2021 (Lincoln, Neb.) – Authorities have been notified about an inmate missing from the Community Corrections Center – Lincoln (CCC-L). Zackery Carlstrom #212038 left the facility early this morning, having jumped over a fence. Staff responded to his last known location, as indicated by the electronic monitoring device he was wearing, but neither he nor the device were located.
